This is one an old fire LT gave me when I first started. I don’t love it, but it’s functional and I never felt like I had to go back and edit it. It’s sized to fit 2 per page in landscape. I’d print them double sided and fold them inside one of these before we switched to ESO and now I just do all my notes in the tough book app.
I prefer the more generic ones because having a checkbox or lost for every possible patient condition or pertinent negative would fill two pages. If it’s a fall, I already know I need to ask about LOC and thinners and if they got dizzy prior, so I’ll not those under the HPI section. If it’s a crash I can use that space for other information and I’m not having to parse the entire document for what’s actually important to the call for a handoff Vs what’s useless
