If you don't mind it being expensive and kind of goofy looking the FIBARO "The Button" fits all your other stated criteria.

Personally I hated how goofy the Fibaro looks so previously I bought a GoControl WA00Z-1 and like it a lot. However, those don't seem to be available anywhere I could find atm. Curious to see if anyone else has another good option that is currently available.
