I got this one from Amazon a few months back and it's been working flawlessly. But if haven't already, hit up /u/kjholoday as he's giving his away.
The replacement power brick arrived, and is working so far (at least for the less-than-an-hour I've tested it). It does make some noticeable fan-like noise, but easy to drown out with actual gameplay/movie sound.

This one. But it doesn't seem like a power supply issue to me. The issue persists whether or not the controller is plugged in so it's not a problem of power draw from the controller, and the audio does play through HDMI so the console is still processing audio.
