It's a design flaw with the joysticks on the knuckles. Regularly using the thumb press will cause the joysticks to start to drift. They'll replace them while you're under warranty, but it's best avoided.
I used plastic conduit to sheathe the replacement cable and keep it from getting sharply bent. It's likely overkill, but I use it because I don't have a pulley system.
There's a hard plastic loop on the rear of the headband that is used for strain relief for the cable, but it's easy to pull the cable in a direction that bends it sharply against it. So I popped it off with a screwdriver without damaging it, and replaced it with a velcro cable management band that gives it some strain relief without being inflexible.
